Admission to Apple Rehab:

Thank you for choosing Apple Rehab. We understand that it is a difficult decision to place your loved one in a Skilled Nursing Facility. Be assured that our staff is educated, dedicated and caring, and will make the transition as easy for you and your family.

Here is some information that you should know about our admission process:

  • We admit 7 days a week.

  • Our Rehab Department provides service 7 days a week.

  • We offer Private and Semi-private rooms.

  • We work closely with hospitals and/or other health care providers to ensure the transition is smooth into the facility.

  • Our Admission staff will take time in explaining the Admission paperwork and our guidelines.
